Required Documents

The following items are REQUIRED to enroll.

• Parent/guardian MUST show a valid government issued photo ID

  Students may be enrolled ONLY by the birth parent or legal guardian (must be court or affidavit approved) who is a resident in the Tulsa Public School district.

  Please call 918-746-7500 if guardianship or residency documentation exceptions may apply to your current situation.

• Acceptable proof of address

  • A full page current (past 45 days), original electric, gas or water bill reflecting the service address. We cannot accept utility company cut-off or late notices.
  • A current, original lease agreement/house contract reflecting property address, tenant names, agent’s name, address and phone number will be required.
  • A bundled/cable phone service bill reflecting the service address. We cannot accept company cut-off or late notices.

  All items must be in the parents’ or legal guardians’ name.

  Note: The following DO NOT meet requirements for proof of address:

  • Driver’s license
  • DHS/IRS letters
  • Bank statements
  • Checks
  • Mail or insurance statements
  • Cellular phone bills or
  • Written statements from a homeowner

• Child’s certified birth certificate

  We cannot accept hospital certificates.

  Age requirements

  Grade level	Age on or before September 1
  Pre-K	4 years old
  Kindergarten	5 years old
  1st Grade	6 years old

• Immunization and vision screening records

  All series must be either complete or in progress. The district provides immunizations for students who qualify for the State Vaccine for Children’s Program. Results of any vision screening completed during the past 12 months are requested for grades KG, 1st and 3rd. Vision and hearing screenings are provided by the district.

• These items are REQUESTED but not required.

  Student’s social security number

  Other information:

  • Certificate of Degree of Indian Blood (CDIB) card
  • Cumulative school records
  • Transcripts for secondary students
  • School or state test records
  • Special Education (IEP) records
  • Court-appointed custody documents, if applicable
  • Name and address of previous school attended, and
  • Telephone numbers for student’s doctors and emergency contacts.
